The Assisted Living Division of The New Jewish Home is seeking a per diem Registered Dietitian (RD) to join our team! The RD will work onsite 7.5 hours per week or 15 hours per month. The schedule that will be given is weekdays, Monday to Friday, alternate days may be needed for resident care plans and food committee meetings The location is 2553 University Avenue, Bronx, NY 10468. Job Summary: + Provides nutrition assessment/clinical services to residents in the assisted living facility. + Handles medical record documentation, MDS 3.0 completion and resident/patient/family education. + Participates in the interdisciplinary care plan meetings, and meal rounds. Requirements: + Registered Dietitian. + Two years clinical experience in long term care preferred
