Sodexo is seeking a full-time Registered Dietitian for UH Connor Whole Health. This clinician will be responsible for working collaboratively with a multidisciplinary team to coordinate and manage the care of patients seeking assistance from the Connor Whole Health practice, which includes Lifestyle and Integrative Medicine experts. This role is responsible for developing and leading shared medical appointments/group medical nutrition therapy, conducting initial consultations and evaluations of patients, developing tailored health plans, and providing guidance for patients or providers on programs and services that may be the most beneficial to support the patient with lifestyle changes that can improve health and well-being. This role will also contribute to the system-wide Food As Medicine initiatives.
What You'll Do Develop tailored health plans, assist patients with implementing health plans and coordinate care.Educate patients and families regarding Connor Whole Health concepts, programs, and services.Develop, organize, and lead shared medical appointments using Connor Group Medical Visit model.Develop, implement, and evaluate comprehensive patient education programs and services that support Lifestyle Medicine. and Culinary Medicine.Participate in the development and execution of quality assessment and performance improvement programs and projects. Support planning of and participate in research projects.Collect and analyze data for ongoing clinical research projects. Qualifications & RequirementsMinimum Education Requirement:
Master's degree with completion of required coursework and supervised practice as dictated by the Accreditation Council for Education in Nutrition and Dietetics (ACEND)
For those eligible for the registration exam prior to 1/1/24:
Bachelor's degree with completion of required coursework and supervised practice as dictated by the Accreditation Council for Education in Nutrition and Dietetics (ACEND)
Credentials Requirement:
Meets eligibility requirements for Registered Dietitian Nutritionist (RD/RDN) by the Commission on Dietetic Registration (CDR) and will become registered within 120 days of hire. In some states and/or facilities the RD/RDN credentials is required at the time of hire
Certification/licensure as required by state(s) of practice. In some states and/or facilities state certification/licensure is required at the time of hire
Minimum Functional Experience - 3 years experienced practitioner able to work with minimal supervision
