We have an exciting opportunity for a Outreach Recovery Worker to join our team who has the skills and experience to assess and engage with our service users, ensuring that they access the right intervention at the right time, in a way that best meets their needs.  

 

Location: Reading

Hours: Full Time, 37.5 per week

Salary Range: £27,861.26 - £32,002.35 (Based on full time hours, pro rata for part time) 

Contract type: Fixed Term until 31st March 2026

 

* Full-time hours at Change Grow Live are 37.5 hours per week. For part-time roles, the salary and payments will be pro rata based on contracted hours.

Responsibilities

About the role: 

 

Establish effective communication and referral pathways those individuals who are homeless and in contact with other health and care agenciesAttend relevant multi-disciplinary team meetings supporting links to health and social care services, peer support networks, community assets and mutual aidWork closely with the local authority, Health & Social care sector, partners and any provision linked to Homelessness and improve joint workingLiaise with other services involved in the care and support of an individual to ensure appropriate and secure transfer of all care needsCoordinate the transfer of any accommodated out of area individual. This may include referral to and liaison with their new drug and alcohol service and pharmacy and
provision of bridging prescription until an appointment and prescription is providedIncrease and enhance partnership working with primary care, mental health services, housing and other voluntary organisations who are supporting rough sleepers

 

About you: 

 

Have a strong understanding of substance misuse issues and experience of working within a similar field Be an excellent communicator, both verbal and written with good IT skills Proactive and able to work on own initiative with excellent time management and prioritising skills Have a good working knowledge of mental health interventions, services, and good practice Be a supportive team player with strong interpersonal skills with the ability to work in partnership with a wide range of professionals, agencies, and internal/external stakeholders Ability to manage change successfully in a way that prioritises the needs of service users Understand the importance of information governance processes and commit to follow and apply all necessary safeguards Seek out learning opportunities to improve and broaden your professional knowledge and skills and to contribute and oversee the learning and development of others
