Summary/Objective: Leads, guides and directs the quality assessment and assurance program and performance improvement (QAPI) activities of the Home Health and Hospice programs in accordance with local, state and federal regulations, standards and established agency policies and procedures.

Essential Functions 

Identifies, develops, and evaluates, in conjunction with the Hospice and Home Health Clinical Directors, the agency’s key performance improvement indicators in accordance with current state and federal laws and regulations. Tracks, evaluates and reports key performance indicators to organizational leadership related to outcomes and regulatory compliance. Develops and implements the organization's QA&A and QAPI activities including process evaluation, data collection and analysis, and ongoing monitoring systems: Assist the Clinical Directors to plan QAPI meetings to discuss quality priorities and develop solutions with organization leaders. Report quality activities with the committee to identify and correct deficiencies. Work with agency leadership to establish goals and thresholds for performance improvement. Identifying and prioritize quality deficiencies. Analyze root cause of identified systemic quality deficiencies. Monitoring and evaluating the effectiveness of performance improvement activities throughout the organization. Maintains current tracking systems to monitor key outcomes. Recommend changes or additional systems as needed for identified deficiencies specific to the organization’s ongoing needs. Coordinates and reports organizational QA&A outcome data monitoring through QAPI committee and as required by leadership. Collects and collaborates with Clinical Directors to develop, monitor, and report key performance indicators.Clinical Operations:Assist Clinical Directors to monitor clinical operations through joint visits with staff to identify areas for improvement and implement necessary changes.Review OASIS and HIS assessment tools for accuracy and report findings to Clinical Directors for any needed staff education to meet metric goals.Assists the Clinical Directors in the onboarding process to educate new staff on QAPI process and agency metrics.Quality and Compliance:Ensure compliance with all applicable local, state and federal regulations.Complete review of OASIS and HIS assessment tools and other clinical documentation for accuracy according to CMS standards and submit monthly data required through iQIES platform.Assist Clinical Directors as needed with audits of clinical documentation to ensure accuracy, completeness, and compliance.Maintains a commitment to quality assessment and performance improvement. Develops and implements a process to ensure care and services meet threshold quality goals as established by organization leadership.Reasonable accommodation may be made to enable individuals with disabilities to perform the essential functions. 
Work Environment  The responsibilities of this position may involve physical activities including standing, lifting, bending, stooping, pushing, pulling and twisting. The employee must be a licensed driver with an automobile that is in good working order and insured in accordance with the organization requirements. The employee may experience a change in work conditions related to weather issues, traveling in rain, snow, etc., and must be able to adapt to the work environment to meet patient care needs. Physical Demands  The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job.

Education and Experience
Graduate of an accredited school of nursing with Bachelor Degree in Nursing required.Current State of Wisconsin nursing licensureMinimum of 2 years of experience in a clinical role, with at least 1 years in QA&A/QAPI role.Home Health and Hospice experience preferred.Skills, Knowledge and Abilities: Able to relate positively, effectively, and appropriately with patients, families, community members, and providers.  Able to read, write, speak and understand English. Meets all health requirements, as required by law.  Demonstrates basic computer knowledge and ability with an aptitude to learn company applications.
