Are you a Qualified Adult Social Worker ready to make a real impact in a Hospital Discharge setting? We’re working with a forward-thinking local authority in Merseyside looking for experienced professionals to join their dedicated Adult Services team.

 📍About the Role:
You’ll play a key role in supporting safe and effective hospital discharges, ensuring the needs of vulnerable adults are met while reducing risk and promoting independence.
Working in line with social care legislation, you’ll carry out strength-based assessments, create personalised care plans, and lead interventions to secure the best outcomes. You’ll collaborate closely with hospitals, community services, and families—putting people at the heart of your practice.

✅ What We’re Looking For:
We’re seeking confident, compassionate professionals who bring:
Social Work England registration4+ years’ experience in Adult Social Care (ideally 2–3 in hospital discharge or similar)A SW, CSS, DipSW or Social Work DegreeA commitment to best practice, dignity, and empowermentStrong skills in mediation, assessment, and multi-agency collaborationKnowledge of current legislation and safeguarding frameworks
💼 In This Role, You Will:
Manage a caseload of complex dischargesCarry out risk assessments and devise care plansUse specialist communication skills to achieve the best outcomesProvide guidance and mentoring to unqualified staff and students
