Summary

Job Summary: The Psychosocial Health Navigator is a professional who has specialized training and expertise in navigating the health care system and resources for high risk individuals in the community. Services provided include but are not limited to supportive counseling, education, information and referrals to community resources and support groups, and working with the health care team and patient's physician(s) to develop a service plan. The PHN works with the clinical team to identify individuals who have complex health needs. The PHN actively manages the patient's care in coordination with the patient's physician and other service providers as well provides resources and support, and educates and empowers the patient/family to take an active role in managing the day-to-day challenges of the patient's individual disease process(es) in order to increase health & well-being.   Requirements: * Licensed Clinical Professional Counselor - Current Maryland license or eligibility to obtain Maryland license * Licensed Certified Social Worker - LCSW - Current Maryland license or eligibility to obtain Maryland license
