Project worker 2 -Family welfare in GP surgeries

We have an exciting opportunity for 4 project workers to join a new pilot service within the Black Country area, working in partnership with GP surgeries.

High numbers of children present to primary care for challenges that require a social, rather than a clinical, solution. Families in the most deprived areas, experiencing multiple disadvantages and health inequalities struggle to access the support they need due to fragmented services, limited resources, and systemic barriers. 

This service is aimed at supporting families with overlapping health and social needs who may benefit from coordinated support. This includes parents pre-birth and families with children aged 0-19 or up to 25 for those with additional support needs including neurodiversity and SEND.

The role of the Project Worker is to work as the ‘glue' to provide case management support, follow up, and signposting to wider support networks across health, social care etc. The aim is to positively engage with families before crises emerge. The role will require active outreach,  including home visits and direct engagement with families.

All available roles are for 37 hours per week. 

Funding in place until end of March 2027

Posts available in the Smethwick (Sandwell) or Ladywood (Birmingham) areas, co-located with local GP's and integrated to local Family Hub offers
