Job Summary:Program Manager of the Case Management Program is responsible for monitoring, overseeing, coordinating and managing the quantity and quality of services provided by HIV case management and housing teams. Ensure all operations meet agency and funding source standards. This position will also maintain a small caseload of individuals living with HIV/AIDS.

Job Duties/Responsibilities:
Direct Care & Coordination of Services
• Perform network and outreach in collaboration with local communities and organizations to identify HIV positive populations in need of medical and supportive interventions/services.
• Facilitate weekly Case Management Team Meetings, by developing agendas that may include but are not limited to announcements, program reminders, client updates, and other relevant topics. As well as documentation and follow-up.
• Manage/document and oversee operations of the BAMSI Community Food Pantry provided in partnership with the Greater Boston Food Bank (GBFB) and other entities. Including inventory, ordering and picking up of food, and distribution of food to persons served.
Perform case management duties including but not limited to
▪ Generate innovative ideas and strategies to identify and attract new clients.
▪ Assist with connecting persons served to medical care, insurance enrollment, medical appointments and other appropriate social, mental health and financial services.
▪ Develop and implement creative outreach strategies to engage clients lacking contact information, such as phone numbers and addresses.
▪ Perform ongoing needs assessments and care planning with persons served.
▪ Develop and provide appropriate supportive referrals in the persons served. identified and/or focused area of need.
▪ Provide transportation, information and education, and assistance with accessing medical and community services.
▪ Conduct home visits and transport persons served to and from appointments (i.e. legal, immigration, social services, medical, etc.) as needed and/or assigned by supervisor.
▪ Screen, assess, and counsel individuals who feel they are at risk of HIV/HCV/STI infection or opioid overdose. ▪ Assist harm reduction staff in the delivery of new positive HIV test results by providing comprehensive overviews of available services and offering appropriate support to clients.
▪ Collect specimens (via venous blood draw, finger stick phlebotomy, oral swab, and/or urine specimen) for HIV/HCV/STI tests from at-risk individuals who feel that they are at risk.
▪ Fulfill charting and data collection duties including training and successful use of CareWare or other EHR.
Community Connecting
• Demonstrate comfort and cultural competency in working with priority populations, including but not limited to individuals living with HIV/AIDS, unhoused individuals, and those with substance use disorder or actively using substances.
• Perform community education services such as community fairs/meetings whose focus will be health issues and access to service for people in the Brockton area.
• Establish and maintain a good rapport and collaborative relationships with staff, persons served and other program-related individuals.
• Attend meetings, conferences, and trainings to expand knowledge of HIV disease and related topics (STI’s, Hepatitis, substance abuse, harm reduction, etc.) and be current on new community resources and evidence-based practices as required by supervisor or designee.
Staff Management & Development
• Provide regular, documented administrative supervision to staff (minimum monthly for established team members and at least weekly during the orientation period for new staff members) in accordance with assignments from the Senior Director of HIV Services. Additionally, facilitate staff training and professional development initiatives as directed.
Quality Management
• Review persons served records to assure up-to-date record keeping in accordance with program and agency policy.
• Review data in CareWare System to ensure the program meets both qualitative and quantitative outcomes.
• Review and maintain client voucher and account systems.
• Ensure case management team knows, understands, and adheres to agency/program policies and procedures.
• Perform duties consistent with agency/program policies and procedures. Execute duties to reflect reasonable safety standards. Universal/standard precautions must be utilized, and training obtained in areas that constitute risk.
Other
• Perform other related work duties as needed or as required by supervisor or designee.
Job Requirements
• Supervisory experience is required.
• Case management or care coordination experience required
• Demonstrated ability to provide bilingual case management in priority populations including, but not limited to, Haitian Creole, Cape Verdean Creole, Portuguese, and Spanish is strongly preferred.
• A firsthand understanding of issues related to living with HIV/AIDS is strongly desired.
• Knowledge of Harm Reduction principles and practices required
• Demonstrate a level of comfort working with diverse populations including people of color, people who use drugs, people experiencing homelessness, sex workers, LGBTQ+, and/or people recently released from prison.
• Persons with lived experience, POC and LGBTQ+-identified individuals strongly encouraged to apply.
• The ability to work both independently and in a team, environment is required.
• Excellent written and oral communication skills are required.
• Excellent organizational, written, and oral communication skills required.
• Demonstrated experience providing culturally competent and sensitive care.
• Proficiency in Microsoft Platforms including Word, Teams, and Outlook is required.
• Must have valid driver’s license and own means of transportation.
• Resident of or familiarity with the Brockton Area preferred. Education/Licensure Requirements
• Bachelor's or associate degree in human service field preferred; Degree substituted with a minimum of 3 years' experience in related field
Physical Requirements: Work is mostly sedentary and is performed in an office environment and at drop in center and requires the ability to operate standard office equipment and keyboards. Repetitive and substantial movements of the wrists, hands, and/or fingers. Ability to travel to meet with persons served in their homes. Must have the ability to move items weighing up to 40 lbs. from floor level to approximately 4 feet above floor level. Must be able to ascend and descend staircases while moving items weighing up to 20lbs.
