Shift Worked: Monday - Friday [Varied Hours] Weekly Hours: 37.5 FLSA Status: Non Exempt Pay range: $28.20 - $30.76POSITION SUMMARY:
Responsible for the development, implementation, and monitoring of the program plans for people receiving services and for ensuring that all people receiving services are afforded opportunities and/or activities that promote individualization, independency, inclusion, and productivity. Responsible for coordinating services with all Agency personnel, as well as outside service providers.
POSITION DUTIES:
Operations:Actively promote the mission, vision and value of CP unlimited.Ensure that residential programs promote person-centered approach in service planning and delivery.
Develop, implement and monitors the program plan for each person receiving services.
Chair and participate in all annual, semi-annual and special team conferences for all people receiving services.
Complete all required documentation (i.e., minutes, program plans, and annual updates for each person receiving services). Ensures monthly notes are completed correctly and in a timely manner and that the records are maintained accurately for all people who receive services.
Review data and provide feedback and training to the team to ensure that service plans are being implemented as planned.
Conduct periodic site visits at least monthly or as needed.
Maintain systems, policies, and protocols that represent best practices
Participate in the new people who reside in our programs intake and screening process.
Review Life Plans to ensure that supports and services are accurately reflected.
Coordinate clinical meetings at least monthly.
Complete PC Reports weekly.
Compliance
Ensure compliance with al current laws, regulations and industry standards
Responsible for completing all training as assigned
Budget and Fiscal Responsibilities
Provide oversight of financial systems related to the people we support and program expenditures including the personal funds of the people we support to ensure that balances do not exceed CPU guidelines
Leadership and Supervision
Possess strong commitment to team environment dynamics with the ability to contribute expertise and leadership at appropriate times
Participate and contribute to Department meetings and trainings.
Incident Management and Prevention
Assist and cooperate with agency and Justice center investigative process related to incidents
Other Duties
Promote self-advocacy for people that reside in our program
Attend all mandated trainings
All other related duties as assigned.
REQUIREMENTS
BA/BS in related human services field.
Minimum 1 year experience in developmental disabilities and/or regulatory compliance.
Thorough knowledge of City, State and Federal regulations governing the operation of intermediate care facilities, individual residential alternatives, day habilitation programs, supported employment.
Performs all job responsibilities in full compliance with all applicable laws, rules, regulations, policies and procedures. Adheres to the agency’s corporate compliance program, including the code of business conduct, NYS and federal false claims acts, whistleblower protections and mandatory reporting. Completes corporate compliance training at least annually. Fully cooperates in all corporate compliance investigations and reviews.
Excellent verbal and written communication skills.
Ability to work flexible hours and/or days as necessary.
