Physician Affiliate Group of New York (PAGNY) is adding a Primary Care Physician - Sickle Cell Disease to the group at NYC Health + Hospitals/Harlem. The Adult Clinic at NYC Health + Hospitals/Harlem is designed to promote wellness and prevent disease and disability. Our physicians are nationally recognized experts in their fields. Special programs/support groups for those with chronic illness are held to provide additional help for diabetes, cancer, amputee education & Alzheimer’s/Dementia. The hospital maintains an academic affiliation with Columbia University Vagelos College of Physicians and Surgeons.

Opportunity Details:

This physician will provide comprehensive and holistic primary care for patients with sickle cell disease, including pain management and infusion treatments. As a patient-centered medical home, routine clinic hours will include at least one evening per week and one Saturday per month. Overnight telephone call will be shared across facilities. Primary Care practice will build over time; target panel size approximately 1,000 patients with sickle cell disease. The physician will see general primary care patients while building the sickle cell practice. Volume approximately 20 patients per day.
