Post-Discharge Call Back Coordinator
*Description* Post Discharge Call-Back Registered Nurse - Danbury Hospital Emergency Department!! RN WITH ED EXPERIENCE WANTED!!! Rare opportunity for an RN with ED (Emergency Department) experience to join this Nationally Recognized Hospital to provide support to our ED. This is a newly created role designed to follow up with ED patients, those seen and not seen and ensure positive outcomes for the health of our patients. Previous experience in the ED is required. This is a FT 40 hours with hours accommodating our patients but are approximately Mon-Fri 830am-5pm. Pay based on full years of experience and is targeting candidates between $70K - $107K per year. Summary: The Post-Discharge Call Back Coordinator is responsible for providing follow-up care and support to patients discharged from the Emergency Department. This role involves contacting patients to assess their recovery, address any concerns, and ensure they understand their discharge instructions and follow-up care plans. The call back nurse would attempt to reach at least 75% of treat and release patients within 48 hours, focusing on the most vulnerable populations such as LWBS, pediatrics, pregnant patients, AMA's, or elopements. Responsibilities: * *Patient Follow-Up:* Conducts post-discharge phone calls to patients within 24-48 hours of discharge to assess their condition and recovery progress. * *Care Coordination:* Collaborates with healthcare providers to address any patient concerns or complications that arise post-discharge. * *Education:* Reinforces discharge instructions, medication management, and follow-up care plans to ensure patient compliance and understanding. * *Documentation:* Maintains accurate and detailed records of patient interactions, concerns, and outcomes. * *Support:* Provides emotional support and reassurance to patients and their families during the recovery process. * *Referral:* Identifies patients who require additional medical attention and facilitate referrals to appropriate healthcare services. * *Quality Improvement:* Participates in quality improvement initiatives to enhance post-discharge care and patient satisfaction. * Maintains and Models Nuvance Health Values. * Demonstrates regular, reliable and predictable attendance. * Performs other duties as required.
