The Post-Acute Care Nurse Navigator supports the transition of care for AMI, COPD, HF and Pneumonia patients. The Nurse Navigator provides support and navigation services to patients post discharge across a variety of medical disciplines. They act as a bridge between patients and the healthcare system, helping patients understand their care, overcome barriers to accessing appropriate care, and assist patients in accessing services. The Nurse Navigator is instrumental in guiding patients through the complexities of their diagnosis and treatment. Assists in implementing strategies focusing on improved care transitions, patient education, medication management, and reducing hospital readmission. Effective discharge planning, clear communication and follow-up care is crucial.

Job Requirements

Minimum Education Requirement :

Bachelors of Science in Nursing within 1 year of hire/transfer or enrolled in a BSN program.

Minimum Experience Requirement :

Minimum of five (5) years of nursing experience required. Experience demonstrating ability/skill in patient and staff teaching and education, as well as the ability to use electronic based medical records and Microsoft Office applications.

Minimum License Requirement :

 California RN license. NIHSS, BLS, and ACLS required.

$57.36 - $88.90
