Key responsibilities include quality reporting through Epic and Excel, drafting narratives, and collaborating with Primary Care offices to compile evidence required for PCMH recognition. This role requires strong critical thinking skills to identify areas of opportunity, lead improvement initiatives, and support transformation efforts. In addition to annual reporting and PCMH activities, the Specialist contributes to closing gaps in care to achieve organizational balanced scorecard goals and improve payer STAR ratings. This position is 100% in-person located in Hartford, CT. *Minimum Requirements* *Education* Diploma from an accredited Medical Assistant Program *Experience* 3-5 years of administrative or clinical support preferably in a healthcare setting *Licensure, Certification, Registration* Medical Assisting/EMT/CNA Certificate We take great care of careers.
