Physician Assistant or Nurse Practitioner (APRN) / Colorectal Plainville
In collaboration with attending surgeon this role is responsible for establishing a new clinical program while providing skilled health care to patients in a variety of clinical settings and facilitating clinical management of patients by partnering with clinical nurses and other members of the health care team The scope of responsibility is to assess plan evaluate and treat human responses to actual or potential surgical problems utilizing appropriate practices standards protocols and guidelinesProvides direct patient care for in office consultations including assessment workups triage procedures emergency work ins and other patient care activitiesAssists attending surgeon in the operating room with colorectal proceduresObtains and documents health history performs and documents physical exam and orders and interprets diagnostic testsProvides counseling to patients regarding diagnosis and expected course of treatmentCollaborates with physicians to provide appropriate direct care interventions within scope of practiceCollects organizes records and discusses data as appropriate and uses sound clinical judgment based in best practice standards and evidence based medicinePrescribes pharmaceuticals according to patient needs and conducts appropriate patient follow up outreachEnsures timely retrieval of laboratory and ancillary studies and diagnostic consultation reportsConsults with appropriate internal and external staff to make appropriate referrals for acute chronic and preventive careCoordinates patient care and visits with appropriate clinical and non clinical staffDocuments patient health care files and completes appropriate forms according to HHC MG policies and procedures including data collection and assessment plan based on patient needs and health care statusCarries out clinical interventions according to established plan of care and adapts actions based on changes in patients needsCounselsrsquo patients and their families with concern and empathy while respecting confidentiality and privacyInitiates call backs within hours for urgent care within hours for triage messagesDemonstrates commitment to ongoing education and improvement as evidenced by documentation of attendance at conferences in services seminars and committee meetingMaintains active involvement with HHC MGrsquos APP CouncilContributes to the development of clinical policies standardized procedures and clinical pathwaysPerforms other related duties in support of efficient department operations and superior patient careShift ndash hours weeklyEligibility to be licensed in Connecticut as a Physician Assistant PA or Advanced Practice Registered Nurse APRN is requiredEligibility to be certified as a Physician Assistant PA or National Board Certification as a Family Nurse Practitioner FNP Adult Nurse Practitioner ANP or Adult Gerontology Primary Care Nurse Practitioner AGPCNP is requiredCurrent State of CT PAAPRN license with prescriptive authority and current DEA number attendance to maintain annual mandatory skill updates years of experience in general surgerycolorectal surgery ambulatory surgery or acute care setting for surgical care preferredExcellent clinical management skills and proven ability to work collaboratively in a team environment and take lead when appropriatePositive customer focused approach with a commitment to providing excellent patient careExcellent written and verbal communication skills Ability to communicate in other languages highly desirableAbility to communicate technical information clearly to patients of all agesHighly organized with demonstrated ability to effectively prioritize competing tasks with full autonomyKnowledge of computer systems including experience with medical software IDX preferredAbility to adhere to HHC MG policies and standards of service as well as HIPPA requirements OSHA regulations and Infection Control standardsCommunicates tactfully and effectively
