We are looking for an experienced Advanced Practice Nurse to deliver high-quality patient care in an interdisciplinary environment. This role will involve collaborating with specialists in Interventional Cardiology, Cardiac Surgery, and Advanced Heart Failure. The nurse will manage patients with heart valve disease in both outpatient and inpatient settings, with an emphasis on preoperative, perioperative, and postoperative care.Specific Description
Ambulatory:
•\tAPP will conduct a valve clinic to facilitate heart valve referrals to
interventional cardiology and cardiac surgery, ensuring a comprehensive,
guideline-based, multidisciplinary evaluation.
•\tAPP will facilitate referrals outside the heart valve clinic to allow
comprehensive, multidisciplinary evaluation for all patients with structural
heart disease.
•\tAPP occasionally assists with individual interventional cardiology or cardiac
surgical clinics if needed.
Inpatient:
•\tAPP will perform a valve service with structural and interventional
cardiologists, cardiac surgeons, and advanced heart failure specialists.
•\tDaily rounds on the valve service will be conducted using a multidisciplinary
approach.
•\tAPP will work closely with valve clinic coordinators to ensure seamless
patient care
Key Responsibilities
•\tPatient Assessment and Management Conduct comprehensive
assessments, diagnose, and manage patients with valve heart disease,
including those undergoing surgical and interventional procedures.
•\tPreoperative Care: Evaluate patients preoperatively, optimize medical
therapy, and coordinate perioperative planning.
•\tPerioperative Care: Provide direct care to patients during interventions,
ensuring seamless team transitions.
•\tPostprocedural Care: Manage postoperative recovery, monitor
complications, and facilitate discharge planning.
•\tCollaboration and Communication: Work closely with cardiologists, cardiac
surgeons, anesthesiologists, hospitalists, referring doctors, and other
healthcare professionals to ensure comprehensive care.
•\tEducation and Support: Educate patients and families on heart valve
disease, treatment options, and self-care strategies.
•\tResearch and Quality Improvement: Participate in quality improvement
initiatives, research studies, and evidence-based practice development.
•\tDocumentation and Reporting: Accurately document patient data, maintain
records, and report outcomes.
Requirements
•\tExperience in cardiology, cardiac surgery, or interventional cardiology is
preferred.
•\tStrong knowledge of valve heart disease and surgical and interventional
procedures.
•\tStrong skills in communication, leadership, and teamwork.
•\tCapability to thrive in a fast-paced, dynamic environment.
•\tCurrent licensure and certification in California.

Assesses, diagnoses, plans and implements the medical care of patients in collaboration with a physician. Functions within the full scope of licensure; accountable for the assessment, coordination delivery and evaluation of nursing care, including direct patient care, patient/family education and transitions of care. Delivers holistic and individualized care to all patients in assigned area. Develops, implements, manages an optimal interdisciplinary plan of care that incorporates psychological, sociocultural, spiritual, economic, and life-style factors. Fosters and maintains collaborative relationships between patients, their family/support group, physicians, and other healthcare providers through timely and effective communications. Adheres to entity polices, industry standards, best practices, and applicable laws/regulations and codes to promote a quality, highly reliable patient experience. Engages in continuous growth and development in professional advanced nursing practice. Follows ambulatory care and inpatient standardized procedures, protocols, policy and procedures regulated by the State of California Nurse Practice Act

EDUCATION:
Graduate from an accredited Physician Assistant program
LICENSURES & CERTIFICATIONS:
PA-Physician Assistant
CSRC-Controlled Substance Registered Certification (DEA) (not required for SHSO-Admin (Employee Health)
BLS-Basic Life Support Healthcare Provider
Other - certification based on department requirements
TYPICAL EXPERIENCE:
2 years of recent relevant experience in Cardiothoracic Surgery.
SKILLS AND KNOWLEDGE:
Demonstrates knowledge of patient care guidelines/policy as evidenced by quality of documentation.
Medical knowledge and skills applicable to perform within the scope of practice.
Thorough knowledge of medical terminology, anatomy and physiology.
Working knowledge of electronic health records such as EPIC.
Critical thinking skills as it relates to specialized departments this may include the ability to complete age-appropriate assessments
Communication skills (verbal, written and listening) with ability to work effectively across the organization, especially in situations requiring instructing, persuading, negotiating, consulting and advising.
Technical and analytical skills for analysis and interpretation of test procedures and results in order to determine appropriate correction actions
Interpersonal skills with the ability to work with and collaborate across the organization and with patients and their families as needed
Ability to effectively lead, mentor and/or develop others
Ability to maintain composure during challenging interpersonal interactions

Pay Range is $92.80 to $134.56 / hourThe salary range for this role may vary above or below the posted range as determined by location.
