Join us in our journey as we continue to expand and create a positive impact on the communities we serve! **Job Description** + Assess patients to develop and initiate treatment plans based on the evaluation results + Review the physician's referral (prescription) and the patient's condition and medical records to determine the physical therapy treatment required + Provide direct patient care by established protocols and patient care guidelines + Document findings, progress, and instructions to patients and caregivers + Responsible for patient care performed by assistants and rehabilitation technicians + Complete a written plan of care, develop goals based on evaluation findings, and develop a comprehensive program to attain goals + Implement physical therapy treatment program and provide essential physical therapy treatments + Communicate effectively with patients/caregivers and professional colleagues, including physicians and insurance companies **Qualifications** + Graduate from an accredited college with an APTA curriculum + Current state licensure + New or recent graduates are welcomed and encouraged to apply **Additional Information**
