You'll support seniors in maintaining or regaining independence, ensuring care is personalized and coordinated within an interdisciplinary team. **What You Will Do:** + Perform in-person physical therapy assessments and develop individualized care plans. + Provide restorative and maintenance therapy to frail or elderly participants. + Supervise and mentor Physical Therapy Assistants. + Collaborate with nursing homes, assisted living, and interdisciplinary teams. + Recommend and evaluate durable medical equipment, braces, prosthetics, and safety devices. + Maintain compliance with all federal, state, and internal policies. + Participate in staff training, safety initiatives, and quality assurance programs. **Minimum Qualifications:** + Bachelor’s degree in Physical Therapy from an accredited program. + Active, unencumbered Physical Therapy license in the state of practice. + At least 1 year of experience with elderly or frail populations (or willingness to be trained). + Strong communication, organizational, and documentation skills. + CPR and First Aid Certification (as required). **Position Highlights and Benefits:** + Meaningful work in a collaborative and values-driven environment. + Opportunities for leadership, training, and professional development. + Competitive compensation and benefits package. + 10% regional travel. + Supportive culture focused on excellence and integrity. **Ministry/Facility Information:** St Joseph is part of Trinity Health PACE, a national leader in providing coordinated care to seniors.
