Apply Today! **Job Summary and Qualifications** **Physical Therapist** We are currently looking for a skilled **Physical Therapist** to join our team on a full-time basis. As a Physical Therapist (PT) on our team, you will assess, evaluate, plan, and carry out physical therapy programs to restore maximum reasonable function to our patients. You will work with patients to minimize any disability following disease, injury, or genetically acquired conditions. The Physical Therapist (PT) provides high-quality, patient-centered care by managing the complete process of therapeutic care of an assigned group of patients in coordination with the physician and the patient care team. The Physical Therapist completes patient assessments and participates in developing and implementing individualized care plans focusing on goal achievement, patient engagement, and positive patient experience. The Physical Therapist provides or directs a majority of the patient’s physical therapy to support patients throughout the continuum of care.
