Reporting to the Director of SCN services, the HRSN Specialist will focus conducting standardized screenings to identify unmet health-related social needs among Medicaid members and facilitating connections to appropriate services. The goal of this position is to enhance health equity and improve
outcomes by integrating social care into the healthcare delivery system.
Schedule:
Mon.-Fri. 9am - 5pm
Essential Duties:
Screening and Assessment: Screen individuals to identify unmet social needs such as housing instability, food insecurity, lack of transportation, or challenges with access to education or employment. Navigation and Referral: Assist individuals in navigating available resources and services, referring them to relevant programs and organizations within the Social Care Network (SCN) and external community partners. Case Management and Coordination: Coordinate with various service providers, including those within healthcare settings, community-based organizations (CBOs), and other SCN participants, to ensure individuals receive comprehensive support. Documentation and Data Management: Ensure accurate and timely documentation of screening results, referrals, and interactions is crucial, often using designated data platforms. They may also track data to evaluate the impact of HRSN services. Building Relationships: Developing strong relationships with individuals, families, community partners, and healthcare providers is important for successful service delivery and care coordination. Community Engagement and Outreach: Engage in community outreach activities to educate individuals about HRSN services and resources, and build relationships with stakeholders
