Pharmacy Specialist – Enterprise Antimicrobial Stewardship - Full Time - Detroit

GENERAL SUMMARY:
The Clinical Pharmacy Specialist – Enterprise Antimicrobial Stewardship, will be a part of the Pharmacy Shared Services team. They are responsible for overseeing the safe use of antimicrobial medications in the health system. This will include helping to manage the health system antimicrobial formulary; overseeing antimicrobial related clinical policies and procedures; managing antimicrobial related drug shortages; coordinating with operations at the acute care sites and clinics for how antimicrobial medications are used. They will promote optimal patient safety, outcomes, cost effective care within the system through optimal use of data and analytics.
PRINCIPLE DUTIES & RESPONSIBILITIES:
+ The Pharmacy Specialist, Antimicrobial Stewardship, is a member of Pharmacy Shared Services and reports to the Pharmacy Manager, Clinical Shared Service.
+ The Pharmacy Specialist is a dedicated antimicrobial stewardship pharmacist that will support the Henry Ford Health Enterprise in the following ways.
+ Serve as the Secretary of the Antimicrobial Sub-committee of the Medication Management Committee
+ Oversee the formulary review and addition of all new antimicrobial requests
+ Oversee the enterprise-wide antimicrobial policies, procedures and guidelines, in conjunction with other antimicrobial stewardship pharmacists throughout the system.
+ Manage antimicrobial drug shortages
+ Evaluate cost effective approaches to use of antimicrobials within Henry Ford Health
+ Review medication safety practices and safety reports related to antimicrobials.
+ Provide education materials related to use of antimicrobials (eg, new formulary additions, updates to policies, drug shortage alerts, etc).
+ Identify opportunities for quality improvement and research related to antimicrobial use across Henry Ford Health
+ Optimize the Electronic Health Record to ensure the safe and effective use of antimicrobials and related drug use policies.
+ Answer questions related to antimicrobials that are sent to the Clinical Shared Services team.

EDUCATION/EXPERIENCE REQUIRED:
+ Must hold a degree from an accredited college of pharmacy and must complete a residency training program or have equivalent experience in hospital pharmacy practice
+ Minimum of 3 to 5 years of pharmacist experience or the equivalent training and experience at another institution, preferred
+ Must be able to serve as a health system pharmacy enterprise authority in antimicrobial stewardship
+ Infectious Diseases Pharmacy Specialty Certification (BCIDP) preferred
CERTIFICATIONS/LICENSURES REQUIRED:
+ Must be licensed or eligible for licensure in the State of Michigan. If eligible, licensure must be obtained within 30 days of hire; failure to obtain licensure will result in termination of the Pharmacy Specialist position.
