\n\t\t\t\t\t•\tResponsible for confidentially reviewing medical records to confirm proper documentation is in the chart to support the pharmacy prescription\n•\tResponsible for submitting detailed chart notes and other pertinent documentation to support the authorization request.\n•\tManages the EMR work queue pertaining to pharmacy prescriptions for new request submission and re-submission to the health plans.\n•\tMust be capable of clear communication to all levels: Clinicians, Clinic and Finance Leadership, clinical team members and patients and Pharmacies.\n•\tCommunicates effectively with patients, providers and insurance companies regarding authorization status.\n•\tDocumentation - maintains accurate records of all interactions and documentation related to authorization requests\n•\tConfirms active insurance is on file. May be responsible to obtain benefits as required or needed.\n•\tRequired to confirm if proper pharmacy benefit workflow is intact.\n•\tResponsible for following deferred standard workflow to avoid patient care delays.\n•\tCollaborate with area Lead and Manager to streamline processes.\n\nCommunication Procedures:\nCommunicates with use of created smart phrases for consistent messaging and/or messages in a courteous and professional manner in accordance with performance standards.\n•\tCoordinates and Communicates with Clinics, Patients, Pharmacists and Health Plans.\n\nElectronic Health Record Documentation:\n•\tAccesses EHR to communicate to clinical staff members and/or physicians through referral in-basket message, pools or telephone encounters using SBAR format and/or appropriate smart phrases in accordance with performance standards.\n•\tManages EHR in-basket(s), referral work queues in accordance with performance standards.\n•\tAll other duties as assigned including department-specific functions and responsibilities:\n•\tPerforms other duties as assigned and participates in organization projects as assigned.\n•\tAdheres to safety, P4P’s (if applicable), HIPAA and compliance policies.\n•\tProvides orientation and training to new staff as assigned.

Pay and Benefits
The pay range for this position is $28.00 - $41.50/hr.
