Directs the delivery of pharmacy services.
Verify all prescriptions for appropriateness of therapy, legality, completeness, contraindications, interactions, allergies, adverse drug reactions, lab value abnormalities and follow-up by screening patient’s chart. Resolve any concerns or questions with prescriber prior to dispensing.
Provides anticoagulation monitoring service as described in collaborative practice agreement.
Prepare prescription orders both written and electronic, in a timely manner. These orders must be filled accurately while maintaining all required record keeping and documentation.
Advise patients clearly and accurately regarding their medications and appropriate outcomes and verify patient understanding. This information will include the indication for which the patient is taking the medication, the drug name and strength, how it is to be used and what changes to expect. Responsible for the medication therapy of individual patients (of all ages) by assuring appropriateness of drug therapy, verifying that patients understand their medications and appropriate outcomes of their drug therapy, and managing therapy for selected patients in whom drugs are the primary method of treatment
Documents all clinical activities and interventions accurately and completely.
Provides drug information, drug therapy consultation and staff education relating to drug therapy:
Provide medication information consultations to all patient care providers and research drug topics. Manage and assure appropriateness of drug therapy and make recommendations where appropriate. Manage therapy for selected patients in whom drugs are the primary method of treatment.
Controls utilization of department resources.
Supervises the requisition and maintenance of drugs and supplies for the department. Maintain inventory at appropriate levels. Select appropriate drugs and vaccines per formulary. Assure availability, preparation, and control of medications.
Evaluates overall performance of the department and plan improvements in pharmacy services.
Formulate and evaluate drug utilization reviews. Contribute to the Pharmacy and Therapeutics Committee as needed. Identify and problem solves trouble areas. Reports medication errors and adverse drug reactions to Medical Director, Pharmacy Director, and QI Coordinator utilizing reporting software. Participate in clinical and administrative activities including quality control and quality improvement within the department and clinic.
Supervises support staff within the pharmacy.
Plans, schedules, assigns, directs, and evaluates work of support personnel; assures adequate training of employees. Ensures staff carries out program responsibilities. Provides for locum coverage prior to scheduled leave.
Responsible for recruiting and hiring vacant technician positions.

Education, Certifications, and Licenses Required
Pharm.D. degree plus 4 years of experience of which one year must have been in a supervisory pharmacy role.Direct Hire – unrestricted active licensure as a registered pharmacist in any state.For Commissioned Officers: an unrestricted active license as a registered pharmacist in any state.Basic Life Support for Healthcare Providers (CPR and AED).ACLS certification preferred.PALS certification preferred.Clinical Competency required within 3 months of hire and subsequently every three years
High Risk Competency required within 3 months of hire and subsequently every three years
Manager Competency required within 3 months of hire and subsequently every three years
Experience Required
Candidates licensed prior to 2003 with will be considered with Bachelor of Pharmacy degree plus 6 years of experience, one of which must have been in a supervisory pharmacy role.Knowledge of
Computer applications necessary to perform duties.Drug therapy and the ability to meet the pharmaceutical needs of all patients.Applicable laws and regulations, safety and security issues, and clinic policies and procedures.Skills in
Calculating drug dosages both oral and intravenous and to compound formulations.Ability to
Lift 30-pound boxes.Work independently and handle stress appropriately.Set priorities, make critical decisions, and respond quickly to emergency requests.Work cooperatively within health system and pharmacy staff.Handle frequent interruptions and adapt to changes in workload and work schedule.Position Information:
