Pharmacist Clinical Specialist Hepatology
Description Location: UCHealth UCHlth Anschutz Outpt Pavilion, US:CO:Aurora Department: UCH Ambulatory Care Pharmacy / Hepatology Clinic FTE: Full Time, 1.0, 80.00 hours per pay period (2 weeks) Shift: Days Pay: $62.19 - $87.07 / hour. Pay is dependent on applicant's relevant experience Summary: This position is responsible for the development and implementation of ambulatory clinical pharmacy services and initiatives that optimize the patient care experience and drive organizational performance in the hepatology space. The pharmacist specialist in hepatology will establish a robust clinical pharmacy practice focusing on patient-centered care and supporting medication management, therapy optimization, and certain pharmacy operational aspects of care delivery. This position will also support infectious disease through ambulatory antimicrobial stewardship initiatives and projects. Requirements: + Doctor of Pharmacy from an accredited school of Pharmacy. + State licensure as a Pharmacist and any relevant life support certification as determined at position level. + Minimum Experience: 5 years specialty experience OR PGY1 or clinical fellowship completion with 3 years' pertinent work experience OR Pertinent PGY2 completion. Responsibilities: + Develops and administers drug therapy protocols as a part of an interdisciplinary team. + Identifies high-risk patients and implements evidence-based interventions to optimize medication-related outcomes. + Designs and provides patient education and medication management to patients and their families. Mentors pharmacy staff on areas of expertise. + In conjunction with a patient's healthcare provider, provides, delegates, and coordinates patient care. Develops and leads quality, safety, and affordability-related initiatives at clinic/unit, service line and department levels. + Maintain accountability for utilization of UCHealth outpatient pharmacies for hepatology medications, focusing on medications requiring specialty pharmacy services. + Documents interventions and their outcomes for use in drug therapy quality improvement efforts.
