Summary:
Responsible for verifying, filling and dispensing medication orders including the proper handling, usage, monitoring and storage of drugs in the pharmacy. Maintains standards set by the department and translates policies and procedures into the daily operation of the pharmacy. Acts as an information resource to patients, physicians, nurses, and other healthcare professionals regarding medication therapy and drug-related information and literature. Serves as a preceptor to students, new hires and residents and supervises and directs support personnel. Other duties as assigned.
Responsibilities:
+ Self-directed with minimal need of supervision
+ Supervised activities of technicians
+ Willing to assist in managing emergent shortfalls of staff
+ Willing to assist in coverage for outreach facilities
+ Participates and leads training/orientation of new pharmacists
+ Participates in precepting pharmacy students and residents
+ Participates in orientation of other medical professionals
+ Participated in patient counseling where applicable
+ Proactive in identifying problems and solutions in patient care
+ Provides drug information for physicians, nurses and patients
+ Provides pharmacokinetic drug consults
+ Consults with physicians and other health professionals ensuring appropriateness of medication orders.
+ Appropriate use of clinical decision support rules
+ Performs pharmacotherapy-related interventions to optimize patient care
+ Interprets and reviews medication orders, maintaining patient profiles
+ Treats patients of all age groups and is knowledgeable about developmental needs over the age span and where appropriate, incorporate findings into individualized care planning and is knowledgeable about use of age related equipment.
+ Knowledgeable in compounding IV's, topical/oral compounds and high risk compounds
+ Participates in audit/surveillance programs
+ Participate in codes on units
+ Assists with data collections for Quality Indicators
+ Report Adverse Drug Reactions (ADR)
+ Auto substitution as per policy
+ Monitors handling, storage and distribution of all medications and chemicals
+ Responsible for accurate fill of patient medications
+ Working knowledge of medication order entry and information retrieval
+ Complete working knowledge of Information Services
+ Performs medication reconciliation of patient's home medications
+ Participates in improving CMS quality indicators for pneumonia, heart failure and MI
+ Complete I-Care annually
+ Score: 5=Completed on Time
+ 1=Not completed on Time
+ Non-formulary medications obtained as Formulary Management Policy
+ Identifies and/or documents cost savings initiatives
+ Identify trends in inventory utilization by monitoring resources and maintain basic supply levels
+ Completes at least 15 CEU's annually
+ Score: 5=30 or more CE's annually
+ 4=25-29 CE's annually
+ 3=15-24 CE's annually
+ 1=<15 CE's annually
+ Maintains knowledge base by participating in at least 6 pharmacy staff inservices
+ Committees participation
