Job Overview:
RWJBarnabas Health is looking to add a qualified Per Diem Pharmacist in Somerville, NJ. The Pharmacist fulfills daily operational responsibilities which assure the safe and effective use of medications in our patients.
Qualifications:
Required:
​​​​​Five or six year Bachelor of Science of Doctor of Pharmacy degree from an ACPE accredited US Pharmacy school or Foreign pharmacy school Every third week weekend rotation and must work one summer and one winter holidayPreferred:
Prior hospital, EPIC and Pyxis experienceCertifications and Licenses Required:
Must be a NJ Licensed PharmacistScheduling Requirements

Essential Functions:
Maintains current patient specific information in the prescribed formatChecks all profile entries for accuracy and completenessReviews physician orders for:
Appropriateness of drug, dose, frequency, and route of administrationTherapeutic duplicationAge appropriatenessAllergies or sensitivitiesCurrent or potential impact as evidenced by laboratory valuesFood drug and Drug-drug interactionsOther complicationsCompares new orders with current patient specific information and resolves all drug related problems, in conference with prescribing physician when necessaryProvides pharmacy documentation of interventionsCommunicates and confers with both nursing staff and patients in order to obtain relevant clinical informationTo provide information to support the nurses responsibilities for administering medications, monitoring drug therapy, and providing patient education,When appropriate in order to obtain relevant clinical informationWhen possible, and when appropriated, educates patients regarding medications prescribed at dischargePrioritizes the processing of medication orders according to clinical importance and other relevant factorsEmergency needs are given top priority and fulfilled as soon as possible,The pharmacist supervises assigned technical staff by:
Prioritizing tasks and overseeing preparation and checking all medications prior to dispensing for accuracy and completenessDelegating other appropriate tasks and checking for accuracyCommunicates frequently with the technician and uses effective supervisory skillsPrepares medication when necessary or appropriate, including: chemotherapy, parenteral nutrition, large volume parenterals, IV piggybacks, and extemporaneous compoundingUtilizes sterile technique methods for the preparation of sterile productsProvides solicited and unsolicited drug information and recommendations to prescribing physicians to assure safe, efficacious, and cost-effective drug therapy and documents suggestionsRecommendations include, but are not limited to:Pharmacokinetic consultationParenteral nutrition consultationSwitching from intravenous to oral formulations of medicationsMedication renewalResponds to all Code Blue and Code White alerts; prepares all medications used in the resuscitation attemptAssists in other ways at the direction of the physician in charge,Professional Competency:
