Pharmacist, Casual - Hospital Inpatient Pharmacy
*_SUMMARY:_* We are currently seeking a *Pharmacist*to join our Hospital Inpatient Pharmacy team. This position is casual, no benefits, day/evening rotating shifts and every 4thweekend. *Purpose of this position:*Responsible for the filling and dispensing of medication orders including the proper handling, usage, and storage of drugs in the pharmacy. Maintains standards set by the department and translates policies and procedures into the daily operation of the pharmacy. Responsible for acting as an information resource to patients, physicians, nurses, and other healthcare professionals regarding medication therapy and drug-related information and literature. Plans, organizes, and supervises work schedule of supportive personnel and pharmacists, as required. Supports the values of the organization by demonstrating personal responsibility, respect for self and others, innovation through teamwork, dedication to caring, and excellence in customer service *_RESPONSIBILITIES:_* * Provides Medication Therapy Management and/or disease state management to clinic patients * Evaluates and maintains a patient’s medication profile to ensure that each medication is indicated, effective, safe, and that the patient is adherent with therapy * Follows documentation and billing expectations, as directed by departmental leadership * Completes timely documentation of activities within the electronic medical record as appropriate for specific area of responsibility per policy from the Office of Medical Staff. Documentation is to be completed within 5 business days of an encounter and no later than 14 days following the encounter * Evaluates, interprets, and delivers medication information to patients, nurses, physicians, or other health care practitioners * Performs all responsibilities/duties required by the pharmacy department as defined by the scope of service particular to each clinic to ensure that the unique nature of the client is addressed * Will precept Resident or PharmD student rotations. May coordinate learner rotations * Maintains current knowledge of pharmacotherapy, pathophysiology, and procedures in area of practice * During patient encounter, based on an assessment of the patient's specific medical needs, make interventions with the medical team members and nursing team members to facilitate optimal drug therapy and facilitate the provision of other pharmacy services * Keeps up-to-date with competencies of both the pharmacy department and those required in the clinic of practice * May work under Collaborative Practice and protocols as authorized by the health-system * Complete health-system and departmental requirements on time and accurately (ex.
