**Schedule:** Full Time
**Shift** : Monday - Friday 8:00am to 5:00pm
**Location:** Maitland, FL, Remote one day in office per month
**The role you'll contribute:**
The Performance & Enablement Specialist is a role focused on driving individual provider practice performance through education, practice transformation, and direct interaction with various roles within primary care offices. Reporting to the Manager Performance & Enablement, this role plays a critical part in supporting the achievement of the organization's performance goals within clinically integrated networks, ACOs, and other provider network arrangements, supporting the value-based enterprise of AdventHealth.
**The value you'll bring to the team:**
+ Visit provider practice offices regularly, advising and educating on topics such as population health strategies, practice transformation, clinical integration, incentive programs, quality care gap closure, and network policies.
+ Assist providers and office staff in understanding value-based performance initiatives and provide guidance on enhancing patient care.
+ Apply advanced knowledge to communicate key messages and solutions effectively to providers and office staff.
+ Collaborate with primary care providers and their teams to identify areas for process improvement, care coordination, and patient outcomes enhancement.
+ Support practice change management related to emerging trends in population health, reimbursement models, and patient satisfaction.
+ Demonstrate comprehensive mastery of population health concepts and performance scorecards.
**Qualifications - External**
**The expertise and experience you'll need to succeed** **:**
**Minimum qualifications:**
+ Bachelor's Degree in healthcare business, or related field; or 2 years of related experience.
+ Experience working with physicians or in healthcare settings
**Preferred qualifications:**
+ Advanced certification or formal training in Population Health, Managed Care, or Value-based Care
+ Change Management Training
+ Lean 6 Sigma Green Belt Certification
