*Per Diem - Pharmacist - Specialty *
*Key Functions*
1. Communication
a. Demonstrates strong interpersonal skills and exhibits customer service through showing compassion, dignity and care for patients
b. Maintains appropriate language and volume when interacting with patients and/or patient's family and providers
c. Provides timely communication with providers regarding prescription changes or patient needs, as applicable
2. Clinical Assessment and Counseling
a. Accurately obtains all elements of a patient clinical assessment, including but not limited to an updated home medication list, assessment of medication adherence, and quality of life
b. Assesses appropriateness of specialty medication and monitors for the achievement of the desired clinical outcomes throughout duration of therapy
c. Participates in specialty clinical consults and counsels patients on specialty medications prescribed including, but not limited to potential side effects
3. Documentation
a. Consistently documents the most updated and accurate information regarding the patient's specialty medication use in the electronic health record
b. Documents an initial assessment and reassessment within the electronic health record in compliance with standards and requirements of accreditation bodies such as URAC
c. Maintains an accurate history within the electronic health record of all patient encounters included but not limited to, all phone calls, emails, and in-pharmacy visits

*REQUIREMENTS*
*Education:* Bachelor of Science degree in Pharmacy or Pharm.D. degree from an accredited college of pharmacy.
*Preferred Education:* Doctorate (Professional)
*Certification:* All of the following:
Texas State Board of Pharmacy Pharmacist license within 90 days of job entry.
American Heart Association (AHA) Basic Life Support (BLS) certification within 30 days of job entry.
American Heart Association (AHA) Advanced Cardiac Life Support(ACLS) certification within 90 days of job entry, if working FT in one of following departments: Pharmacy Inpatient (710348), Pharmacy ATC ACB (710355), Pharmacy-West Houston (710657) Pharmacy-The Woodlands (710480).
*Preferred Certification:* Basic Life Support (BLS)
*Experience:* None.
*Preferred Experience:* Prefer 1 year of experience in a hospital pharmacy retail clinic or specialty pharmacy services or ASHP PGY1 Residency training.
*Onsite Presence: * *Is Required*
