The primary responsibility of the Clinical Oncology Access PA is to coordinate the inpatient access for the Department of Medical Oncology. The Oncology Access PA directs and coordinates patient flow for all oncology patients admitted or transferred to BWH/DFCI to support the service. In collaboration with the Medical Director, Directory of Oncology Access, nursing leadership and Chief PAs; he/she also helps facilitate timely and appropriate team and bed assignments for oncology patients from outpatient clinic, outside facilities, emergency department, internal transfers, and direct admissions. Shifts will be 12-hour shifts.

Responsibilities

*Generates a daily bed plan strategy based on the predicted bed needs and available beds for that day. Extensive chart review will be utilized to ensure appropriate selection of team and bed for each patient based on patient need and available resources. Continual review throughout the day of incoming patients and oncology discharges is required

 

*Leads daily interdisciplinary meetings with representation from nursing, care coordination, and oncology service line leadership with the goal to confirm discharges for that day as well as to identify barriers to discharge. Any barriers will be escalated as needed to appropriate departments

 

*Collaborates with nursing leadership to ensure acuity dispersed throughout the oncology pods to optimize nursing and patient care resources

 

*Facilitates communication between inpatient and outpatient teams to ensure safe transitions of care for urgent and emergent admissions

 

*Participates in identified quality improvement projects related to improving communication, transitions, and patient admission workflows

 

*Collaborates and coordinates with patient access services to provide through-put and bed utilization services to enhance the patient experience, while optimizing patient flow.

 

*Communicates with ambulatory practices and outside hospitals to coordinate direct admissions and transfers.

 

*Create detailed overnight plan and provide appropriate handoff to BWH bed planners.

 

The PA is a member of an interdisciplinary care team that requires effective com5munication and collaboration with co-workers at the Institute in addition to collaborating institutions and agencies. The role requires critical thinking and leadership that improves the quality of care and advances medical care within the work setting and throughout the community. The PA takes advantage of formal and informal learning opportunities to strengthen his or her knowledge base in support of the Institute’s Mission, Vision and Core Values.

Qualifications

PA Qualifications:

· NCCPA certified

· Graduate of AAPA/NCCPA accredited Physician Assistant Studies Program

· License as a Physician Assistant in Massachusetts

· Complete 100 hours of CME according to national and stats regulations during each 2 years period of employment
