**Pediatric RN Case Manager**

As a **Pediatric Registered Nurse Case Manager** , you’ll play a critical role in supporting young patients and their families with empathy, clinical excellence, and unwavering compassion. Whether in a family residence, foster home, or transitional setting, you’ll deliver holistic, age-appropriate care throughout complex medical journeys—with sensitivity, skill, and the heart of a caregiver.
**Schedule:** Monday through Friday, day shift
**Territory:** New Haven to Bridgeport

**What You’ll Do as a Pediatric RN Case Manager:**
+ Provide nursing care that aligns with Elara Caring’s mission and pediatric care philosophy
+ Assess the physical, emotional, developmental, and spiritual needs of pediatric patients and their families
+ Collaborate with physicians and interdisciplinary team members to develop and implement individualized care plans
+ Educate and support families as they care for children with complex medical needs
+ Ensure accurate and timely documentation, communication, and care coordination
+ Deliver skilled nursing interventions such as medication management, symptom monitoring, and caregiver education
+ Advocate for the best outcomes for your patients while honoring their unique family dynamics
**What You Need to Join Us:**
+ RN licensure in Connecticut
+ Graduate of an accredited nursing program (ADN, Diploma, or BSN)
+ Minimum 1 year of recent RN experience in pediatric, home health, critical care, or related setting
+ Pediatric experience strongly preferred
+ CPR certification (pediatric-specific training a plus)
+ Ability to travel within your assigned region (50% local travel)
+ Reliable transportation, valid driver’s license, and insurance
+ Physical ability to lift up to 50-100 lbs and support patients as needed
