Job Overview:
The Registered Nurse Coordinator in the Special Needs Primary Care practice provides guidance for patients and families seeking services by serving as an active liaison between patients/ families, medical sub-specialists, therapies, admissions, insurance companies and community medical providers within a family centered care model. They are responsible for assisting families in finding the proper resources to supplement medical care and quality of life, monitoring family and patient s understanding and compliance with treatment goals, interacting with social services and community agencies, and supporting family through provision of resources and support networks as appropriate.
The RN PCC Peds Practice will function in a team based, collaborative setting to coordinate the care of patients in a family centered manner,The RN PCC Peds Practice will be an active liaison between patients families and the multidisciplinary team including medical subspecialists, therapies, family resources, educational services, insurance companies and external agencies,The RN PCC Peds Practice will help patients families shape and reach goals so that patients may meet their full potential in a medical home based model,The RN PCC Peds Practice will provide care coordination consistent with a Best of Breed medical home,Qualifications:
Required:
Graduation from an accredited Nursing programNJ Registered Nurse LicenseAHA BLS Provider CertificationExcellent verbal and written communication skillsInterpersonal, organization and planning skillsHigh degree of customer sensitivityCognitive analytical and problem-solving skillsAttends to all characteristics, competencies and functions of a PCC outlined on file in deptAmerican Heart Association AHA BLS Provider and PALS certification requiredPreferred:
BSN degreeExperience in care coordination preferred

Coordinates care for children across continuum from intake, outpatient and inpatient servicesCoordinates communication between patient/family, internal and external service providersDemonstrates an understanding of family centered care, patient s medical diagnoses, prognosis, progress and implications for family support, education and goal directed treatments and therapiesProvides triage for families to determine service needsReviews and organizes intake materials and consults with Practice team as neededEngages in advocacy for patient, and foster family's abilities to assume independent skills in seeking and obtaining supports and services for childDemonstrates an understanding of the health care system, managed care and resource utilizationRefer patient and families to community resources to provide access to services such as financial assistance, legal aid, etc.Obtains and manages outcome information for patients in care coordination programAdvocates for the unmet needs of patients / families across the medical, social and financial continumsProvides necessary documentation to external agencies as neededEffectively communicates financial and social needs, barriers, and issues to the multidisciplinary team, physicians and external agenciesIdentifies families at high risk for child neglect or abuse and makes appropriate referralsBe a resource for families to ease them through transitions and hardshipsCompletes proper documentation in a timely manner and provides necessary documentation to external agencies as neededActs to enhance understanding of CSH services and mission through development of relationships with community and government agencies, service providers and referral sourcesOther Duties:
