In this vital role, you will oversee the care management of pediatric members who reside in skilled nursing facilities and those requiring private duty nursing, helping them achieve their highest level of wellness. As the Pediatric Field Care Manager Nurse for Region C (Pasco and Pinellas Counties), you will play a key part in assessing and supporting the medical and social needs of our members. The Pediatric Field Care Manager Nurse employs a variety of strategies, approaches, and techniques to manage the physical, environmental, and psycho-social health needs of members enrolled in Enhanced Care Coordination (ECC). ECC helps to connect children diagnosed with but not limited to Chronic medical conditions, congenital conditions, developmental disabilities, and learning disabilities to care services.

Identifies and resolves barriers that hinder effective care.Ensures member is progressing towards desired outcomes by continuously monitoring patient care through assessments and/or evaluations.Conducts face-to-face visits with members at skilled nursing facilities monthly.  Assists families to coordinate care services while respecting the their right to decide the appropriate care setting for the child.Collaborates with health care providers to develop a treatment plan that aligns the child with the appropriate care services.Improves communication among health care providers, member, parent/guardian, systems, and agencies.Helps to reduce duplication of efforts, ease transitions of care, limit gaps between service providers, and secure transportation or other support / enabling services.

Required Qualifications

Registered Nurse (RN) in the state of Florida with no disciplinary action with a minimum of 2 years Pediatric acute care nursing experience OR Licensed Practical Nurse (LPN) in the state of Florida without disciplinary action with a minimum of 4 years Pediatric acute care nursing experience.Intermediate or better working knowledge of Microsoft Office applications including Word, Excel, and Outlook.ACHA Level II Background Check is required.

Preferred Qualifications

Understanding of AHCA regulations, NCQA and Medicaid guidelines.Managed care and/or case management experience.Certified Case Manager (CCM).Bilingual English/Creole or English/Spanish.

Additional Information:

Workstyle: Field. This is a field position where employees perform their core duties at non-company locations, such as providing services at business partner facilities or prospects' and members' homes.Work Location: must reside in Florida in Pinellas or Pasco Counties, would also consider southern Hernando or northern Hillsborough Counties.Travel: will be required to see members in nursing facilities monthly at a minimum, primarily within Region C (Pasco and Pinellas Counties).Typical work days/hours: Monday – Friday, 8:30 am – 5:00 pm EST

TB screening

This role is considered patient facing and is part of Humana’s Tuberculosis (TB) screening program. If selected for this role, you will be required to be screened for TB.

Driving Statement

This role is part of Humana's driver safety program and therefore requires an individual to have a valid state driver's license and are expected to maintain personal vehicle liability insurance. Individual must carry vehicle insurance in accordance with their residing state minimum required limits, or $25,000 bodily injury per person/$25,000 bodily injury per event /$10,000 for property damage or whichever is higher.

Pay Range

The compensation range below reflects a good faith estimate of starting base pay for full time (40 hours per week) employment at the time of posting. The pay range may be higher or lower based on geographic location and individual pay will vary based on demonstrated job related skills, knowledge, experience, education, certifications, etc.


 

$71,100 - $97,800 per year


 

This job is eligible for a bonus incentive plan. This incentive opportunity is based upon company and/or individual performance.
