Are you a dedicated and compassionate Community Care Registered Nurse (RN) with a passion for making a positive impact on the lives of individuals with disabilities and/or mental health challenges? Do you possess excellent communication skills and a patient demeanor that allows you to connect with individuals in need of support? If so, Mainstay Supportive Housing & Home Care, Inc. invites you to step into the role of a Per Diem Registered Nurse to support clients in the Newton and Boston areas. You will play a vital role in helping population maintain their independence and well-being within their homes and communities.
This is a per diem position supporting community health programs, with an estimated time commitment of 5-10 hours per week.
What You'll Do: As a Per Diem Community Care Registered Nurse with Mainstay, you'll be at the forefront of delivering top-tier support services to seniors who require support with their activities of daily living. Your responsibilities will include:
+ Collaborating with the program manger and support staff to provide high-quality non-clinical care.
+ Conducting in-depth assessments of potential participants and monitoring their health status.
+ Building meaningful relationships with clients and caregivers.
+ Ensuring compliance with Mainstay's policies, procedures, and industry regulations.
+ Evaluating and refining personalized care services plans to enhance client well-being.
+ Engaging in regular in-home visits and documenting your interactions and observations.

Qualifications: To excel in this role, you'll need:
+ A Bachelor's Degree in Nursing (strongly preferred).
+ 1-2 years of experience in home care or home-based services (preferred).
+ Prior experience with Adult Foster Care (AFC) and/or Group Adult Foster Care (GAFC) nursing is helpful.
+ A valid RN License in MA and a valid Driver's License.
+ Strong skills in electronic-based documentation.
+ Empathy, patience, and exceptional communication skills.
