PCMH and Incentive Coordinator\n

Purpose: Responsible for the day-to-day oversight of Cayuga Medical Associates' Patient Centered Medical Home (PCMH) program, which spans all primary care locations. Supports the design, initiation and facilitation of PCMH workflow processes throughout each practice, monitors PCMH compliance, coordinate training activities related to PCMH data collection, patient tracking, policy and procedure development, and workflow changes. Works closely with the Electronic Health Records team relative to PCMH requirements and sustainability. In addition to PCMH, coordinates efforts with payers to help close gaps in care in order to improve the health of our communities and maximize reimbursement.\n

Essential Job Functions\n\nOversees the implementation of a PCMH recognition program for Cayuga Primary Care practices.\nAttains expert knowledge of NCQA PCMH standards, policies, processes and application requirements. Advises management of changes to continuously improve content and delivery.\nDevelops new policies and procedures necessary for PCMH implementation. Evaluates existing policies for compliance with the PCMH model.\nDevelops and implements best practices to improve workflow related to PCMH quality incentives and practice transformations.\nOversees the collection, integration and evaluation of diverse data related to clinical and operational performance and medical home transformation activities.\nWorks collaboratively with other staff members in regard the PCMH model. Evaluates the performance of care teams and makes recommendations for changes. Provides input to the care coordination process. Participates in the quality improvement committee to ensure PCMH issues are included in the overall quality plan.\nResearches, gathers and reports health related data to meet NCQA requirements. Provides regular program updates and outcomes to CMA leadership.\nUpdates documents and participates in the submission process for grants and accrediting agencies.\nUtilize project management skills for tasks such as large mailings of reminders to patients.\nInteract with insurance company representatives and utilize websites to import current patient data and reports to determine patients' current status in completion of preventive activities.\nMaintain knowledge of incentive programs offered by payers and the requirements for payment.\nPrepare progress reports to ensure incentive program requirements are met.\nLead and attend practice PCMH meetings to review progress with the practice's PCMH team.\n\n

Education and Experience\n\nAssociate's degree preferred\nBachelor's degree preferred\nSome PCMH experience is required. Six months experience using an electronic health record system preferred. PCMH Certified Content Expert preferred. Experience with accreditation processes helpful.\n\n

Location\n\nPosition will begin as a fully onsite position. Once the team member is oriented and can successfully work on their own hybrid functions may be approved by the Director of Practice Operations for Primary Care and Women's Health. If approved, the team member must work a minimum of three days per week onsite at a Cayuga Primary Care location.\n\n
