Major Duties include but are not limited to: 

Coordinates new referral for heart transplant evaluation, either as an inpatient or outpatient basis.   Providing information to prospective patients and their referring physicians, arranging consultations, diagnostic procedures, and coordinating follow up appointments.   Communication and coordination of patient care with heart failure nurse specialist, transplant cardiologist, inpatient team members, heart transplant nurse coordinators, cardiac surgeons, bed control, admissions, case managers, and utilization review.   Assisting the Cardiomyopathy Administrative Supervisor with obtaining insurance authorizations and working with other administrative team members to facilitate the complex transplant evaluation process.   Entering patients into the heart transplant database, maintaining the registry of referring physicians, facilitating communication with referring physicians of diagnostic testing and evaluation status, and triaging phone calls.   Type and proofread correspondence. Excellent written and communications skills.   Strong organizational skills and ability to operate efficiently in an environment of frequent interruptions/distractions. Salary Range:  $30.36 – $43.49/hourly
