Description

In this role you will provide administrative and patient-centered support to the Dementia Care Specialists (DCSs) and their patients/caregivers within the UCLA Alzheimer’s and Dementia Care Program. You will serves as a key liaison between patients, caregivers, DCSs, and referring physicians. Responsibilities include:

Assisting patients in navigating services within the health system Facilitating timely communication among care team members  Coordinating referrals to appropriate community and clinical resources.  Follow-up through telephone outreach and accurate documentation in EPIC/CareConnect.  Being professional, compassionate, and have strong organizational skills to support the delivery of high-quality dementia care. Salary Range: $30.36 – $ 43.49/hourly
