The Patient Placement Assistant plays a vital role in coordinating all patient placement activities throughout the facility. This position partners closely with physicians, nursing staff, and various departments to ensure patients move efficiently along the continuum of care, promoting optimal clinical outcomes and high levels of customer satisfaction.

Responsibilities include entering and maintaining accurate patient information in the electronic health record system, facilitating timely communication between key stakeholders, and supporting smooth and efficient patient transfers within the hospital.

Qualifications

Requirements:

High school diploma or equivalent required

6 months to 1 year of experience in patient-related clerical or customer service roles

Experience with medical terminology, medical office procedures, or patient registration required

Strong interpersonal and communication skills

Proven ability to work independently and manage tasks with minimal supervision

Responsibilities

Collaborates with nursing staff and other departments to coordinate and assist with efficient patient flow and placement from all sources

Enters patient information accurately into the electronic system (Memorial Access) and collects basic patient data for the RN

Maintains and updates electronic systems with current and pertinent patient information, ensuring accurate and timely documentation

Utilizes communication tools such as pagers and phones to coordinate and ensure smooth patient placement and transitions

Answers incoming phone calls and processes messages received via fax and printer

Communicates and collaborates effectively with nursing units, physicians, departments, and outlying facilities to facilitate timely and efficient patient placement

Supports patient flow process improvements and partners with the Patient Placement team to identify and implement quality improvement opportunities

Demonstrates flexibility, organization, and the ability to multitask effectively in high-pressure situations

Understands and adheres to hospital, departmental, and Joint Commission policies and procedures related to patient placement

Demonstrates proficiency in basic computer operations

Performs other related duties as assigned
