Under the direction of the Manager of SMG Ambulatory Care Management, the Patient Outreach Specialist will support transitional care efforts for patients—particularly those requiring hospital and emergency department follow-up, including ACO (Accountable Care Organization) members.
The role involves outreach, scheduling, care plan tracking, and coordination across various systems to ensure continuity and quality of care.
High school diploma or equivalent Experience in a healthcare setting, preferably in Transitional Care/Care Coordination, medical office, or administrative support Proficiency with EHRs and care management platforms (e.g., Meditech, Bamboo, Epic) Proficient in Microsoft Office Suite (Word, Excel, Outlook, PowerPoint) Basic computer literacy, including data entry and navigation of electronic systems. Familiarity with electronic medical records (EMR) systems; ability to interpret, extract, and manage patient data in healthcare environments
