Job Overview:

As part of RWJ Barnabas Health Medical Group, you will play an instrumental role in providing support to practices in achieving a model for improving and managing performance in various Value-Based Programs. The Patient Navigator s main objective is to support the Medical Group to efficiently deliver value to patients and physicians while operating as a high-performing academic medical group within the RWJBH One System-One Family Model. The Patient Navigators will work directly with the Primary Care Service Line to align transformation activities and allow for quick implementation and dissemination of information to practices.

Qualifications:

Required:

Minimum associate s degree in healthcare administration, business, or a related field 3-5 years experience in practice management, healthcare operations, or a related roleUp to 75% travel within the state of New JerseyMust possess or within 30 days, a valid New Jersey Driver s License and dependable transportationOnsite work with designated practices and limits remote workTravel between practices Knowledge of Medical Terminology Understanding HIPAA and the ability to protect patient information

Preferred:

Bachelor s degree in healthcare administration, business, or a related fieldLEAN or Six Sigma certificationExperience with Epic Knowledge of population health Previous work experience in physician practice

Certifications and Licenses Required:

BLS Certification approved by the American Heart Association Medical Assistant Certification: NHA, NCCT, AMT, AAMA, NCMA, AMCA, AECA, or NAHP

Scheduling Requirements:

Monday Friday 8:00AM-4:00PM or 8:30AM-5:00PM Full-TimeHybrid position with travel required within the Southern region up to 4 days a week

Essential Functions:

Interface with Practices and key members of the Primary Care Service Line, and Physician Leadership Teams coordinating efforts to improve value-based performance within Medical Group practices.Collect and analyze data to provide practice facing teams with actionable targetsIdentify practice opportunities requiring focused effort and close gaps in careReview upcoming value base program opportunities and coordinate participation discussion with Service Line, complete contractual agreementsConduct ongoing education on Epic workflows, Value Based Metrics, and other needs as they ariseHost standing education platforms virtually or in person on a monthly basisCollaborate with the Primary Care Service Line to develop and train detailed practice workflows to enhance value base performanceSuccessfully work with practice manager to assist with change management efforts at the practice level around workflow redesignServe as escalation point when workflow opportunities are identified in implementation practice workflow changes and addressing low performanceManages communication to the practices and practice staff around value-based activitiesProviding coding education and best practice around value-based programs and activities
