UPMC is hiring a Full-Time Patient Navigator, for the REACCH Program to support the PloyClinic Campus in Harrisburg.

Occasional travel for events required.

 

Schedule: Monday through Friday 8:00am – 4:30pm

Requirement of at least 1 weeknight until 6:30pm to support later group or clinic schedules

Provides HIV medical case management per Pennsylvania Case Management Project guidelines for approximately fifty REACCH clients. Functions as a professional, participating member of the entire multidisciplinary REACCH Team, with the goal of lessening total viral load for all of the patients at REACCH. Uses therapeutic interactions and motivational interviewing to coach patients to meet goals. Under the oversight of the Program Coordinator and with accountability to the entire REACCH Team, functions within the clinic and within the community with moderate independence, and as a positive representation of REACCH and of the PinnacleHealth System.

Primary Duties-Medical Case Management:

Maintain a caseload of approximately 40 medical case management clients. Perform and follow-up according to Part B Ryan White medical case management standards for these clients. Link clients to community resources and departmental resources such as mental health care, substance abuse services, housing, etc. Ensure all clients applying for financial assistance meet eligibility and financial requirements. Work one-on-one and in a group setting with clients to become self-advocates along their HIV diagnosis and treatment journeys. Offer a safe space for clients to express their perceived strengths, barriers to treatment, and explore individualized strategies for success. Provide medication reminders and empowerment.

Secondary Duties:

Provide rapid HIV testing on-site at REACCH and off-site in the community. Participate in community outreach & engagement events. Provide follow-up with newly diagnosed clients at UPMC hospitals, off-site outpatient offices, and on-site at the REACCH Program, as needed. Other duties as assigned.
