Patient Flow Registered Nurse (RN)
Position OverviewUnited Health Services (UHS) is seeking a Patient Flow Registered Nurse to support hospital-wide patient movement and care coordination. In this critical role, you will work closely with clinical teams to manage admissions, discharges, and bed placement, ensuring patients receive timely and appropriate care in the right setting. Primary Department, Division, or Unit: Clinical Resources, UHS Hospitals Primary Work Shift: Evening Rotational Regular Scheduled Weekly Hours: 40 Compensation Range: $40.70 - $61.05 per hour, depending on experience ----- Sign-On Incentives: Up to $15,000sign-on bonus for candidates who meet eligibility criteria. Job Responsibilities + Oversees the day to day movement of patients to facilitate the organizational goals of timely patient placement and discharge. + Oversees and monitors the bed tracking board to ensure data is up-to-date and communicates information during bed huddles as needed. + Sets priorities for patient placement by setting priorities for ED admits, obtaining patient arrival times for direct admits, and communicating with Access Care on an ongoing basis as unit situations change. + Facilitates structured daily bed meetings to provide an organized approach to patient placement. + Assures proper placement of patients on units during bed meetings with accurate information including current admits in ED, potential admissions, staffing needs/status, and number of “borders” and by adhering to patient placement policies. + Implements protocols for increased patient census. Initiates “House alerts” and action for overcrowding or gridlock according to policy. + Maintains follow-up action list to validate that agreed upon actions occurred. + Provides direction to the Access Care Staff, Nursing Units, Environmental Services, and the ED to prioritize and determine appropriate placement of patients. Collaborates with nursing units and Environmental Services to prioritize cleaning of rooms by rounding on nursing units to expedite and facilitate room turn around. + Coordinates patient transfers from other facilities to UHS Hospitals. Oversees Transfer Center processes and serves as liaison between UHS Hospitals and sending facility. Seeks feedback related to transfers and process improvements. Manages data captured in LVM System from transfer activity and provides reports to Senior Management and others as requested. + Serves as patient advocate by assuring care rendered is appropriate, timely and confidential.
