As a Home Care Physical Therapist (PT) at Holy Cross Home, you’ll provide skilled physical therapy services to patients in the comfort of their own homes, enabling recovery, improving mobility, and enhancing quality of life. Using advanced technology and your clinical expertise, you’ll assess, plan and manage individualized care that promotes healing and independence.

✅ **Responsibilities:**
+ Assess patients’ functional status and physical therapy needs in their homes.
+ Develop and implement individualized care plans.
+ Provide direct physical therapy services in accordance with the physician’s plan of care.
+ Educate patients, families, and caregivers on exercises and therapy techniques.
✅ **Qualifications:**
+ Current licensure as a Physical Therapist in the State of Maryland
+ Graduate of an accredited physical therapy program.
+ At least one year of clinical experience (home care experience preferred).
+ Valid driver’s license and reliable transportation.
+ Ability to work independently and manage time effectively.
