Hours: Part-time -20hours per week

Monday thru Friday: office hours are 9:00am - 5:00pm

This exciting and challenging position requires good
therapeutic and social skills to work closely with seniors, their
families, medical professionals, hospital and nursing home staffers,
and caregivers.

Responsibilities:

Will be working closely with seniors and there families, care givers, medical professionals to provide case management and assist with their overall healthcare coordination needs. Will be travel to a facility if needed or they will be meeting with patients/families via zoom. Would like for them to participate in conferences as needed and new patients they take on to manage. Will work side by side with an attorney. Responsible for managing referrals for patients depending on patient needs-Assisted living, LTC, SNIF, DME and Homecare.
