PARISH NURSE

Summary
Acts as a liaison from St. Joseph Hospital to the community. Provides general health counseling and health education to parishioners and community. Assesses needs for, develops, and implements health education programs involving parishioners; provides referrals of available healthcare resources to parish and community.
Essential Duties and Responsibilities
+ Supports and promotes the mission and values of Covenant Health Ministry.
+ Liaison to the community through the church parish/ SJH Senior Resource Center/Affiliate; meets with participants to guide them to available healthcare facilities/programs in the community; provides support to families in times of medical crises.
+ Identifies patient needs for referral, advocacy, health counseling and or educational programs; arranges to meet those needs.
+ Plans and provides programs for the churches/community; provides necessary training to involved participants.
+ Prepares and submits ongoing reports of activities/issues to Director Community Ministries. Keeps the Director Community Ministries appraised of pertinent issues.
+ Participates as member of various hospital/ community-based committees, as they relate to the Parish Nurse Program
+ The employee acknowledges the importance of patients and their families as the most important parts of the job whether or not the employee provides direct patient care.
+ The employee is to demonstrate respect and courtesy and assist patients and families with requests or seeks assistance of direct care or other staff on behalf of patients and families.
+ Seeks opportunities to expand professional growth through in-services, continuing education classes, and other work-related activities.
+ Performs miscellaneous duties and/or special projects as assigned to the satisfaction of the Director Community Ministries.
+ May assist with recruitment, training, and coordination of volunteers for Parish Nurse Programs.
+ Other duties as consistent with this role.
Job Requirements
Job Knowledge and Skills
+ A broad knowledge of general nursing required.
+ Excellent verbal communication skills and decision-making ability
Education and Experience
+ NH Registered Nurse license required
+ Basic Cardiac Life Support certification required
+ Bachelor’s Degree in Nursing preferred
+ 2 years’ experience in personal health counseling preferred
