**Description**
Summary:
The Paramedic, Post Acute Svcs. works with designated high risk patients in their home environment patients to provide proactive post acute care and is intended to help improve the overall health and wellness of program participants. Expands access to non-emergent health care through in-home patient visits,, and provides timely collaboration with primary care, provides resources, outreach education to address social determinants of health. Paramedic will consistently perform in compliance with their position description, State protocols (NCOEMS) and with the policies/procedures of the community paramedic program and the post-acute department. Performance is evaluated in cooperation with Director of Care Management and program manager by observation and through review of records and reports.
Responsibilities:
1. Provides proactive care that is intended to help improve the overall health and wellness of program participants. Requires excellent rapport, understanding, and communication with the patients and their family members.
2. Expands access to non-emergent health care and chronic disease management through in-home patient visits and/or community-based services with a goal of producing better health outcomes and reducing visits to the emergency department, and hospital readmissions. Common services will include:
a. Assessment (vital signs, blood pressure, labs such as glucose levels, diagnostic cardiac monitoring, medication compliance and reconciliation, adherence to discharge instructions, etc.),
b. Treatment (wound care, medication administration, post-operative follow-up, etc.), prevention (immunizations, home safety check, health education, etc.),
c. Referral (mental health, medical and social services, etc.).
3. Follows established Standard Operational Guidelines and State treatment Protocols.
4. Works in cooperation with other key stakeholders/key medical providers in order to offer coordinated and integrated care (physician offices, home health agencies, hospice, long term care facilities, public health department, social services, etc.).
5. Maintains thorough and accurate patient care documentation and completes all paperwork in a timely manner.
6. Provides on-site patient care in conjunction with other medical providers to offer non-urgent integrated care such as physician offices, hospice, social services, health department, etc.
7. Functions as a Mobile Integrated Healthcare Specialist with assigned patients and navigates community resources to reduce barriers to care and provide education and access to services available to patient. Must also maintain or exceed Paramedic job description. ???
8. Demonstrates standards of performance (respect, teamwork, communication, compassion) that support and encourage patient satisfaction and principles of service excellence.
9. Builds and advocates for the program. Creates community engagement. Functions as physician and hospital extenders by utilizing advanced protocols/procedures approved by the Medical Director.
10. Follows established lines of authority and responsibility and ensures that service objectives are met and maintained
11. Attends continuing education classes and trains on a regular basis to the satisfaction of the training officer. Subject to regular performance evaluations so that any deficits may be corrected immediately. Attends professional development training at least on an annual basis.
12. Obtains and maintains Community Paramedic Certification
13. Keeps appropriate leaders or others informed of any issues, problems or concerns.
14. Presents and promotes a professional demeanor at all times, establishes high standards of customer service, supports responsibility, pride, and teamwork, and offers reasonable solutions for problems.
15. Demonstrates appropriate interaction with, and provides recommended care and services to all patients.
16. Ensures the delivery of age appropriate, culturally sensitive, and ethical care for patients.
17. Applies knowledge of principles of standard precautions. Routinely uses PPE to avoid risk of exposure to self and others.

Education
· High school diploma or equivalent required.
Licensure/Certification
· Credentialed by the North Carolina Office of Emergency Medical Services and the Medical Care Commission as a Paramedic.
· Community Paramedic Certification required. Must obtain within one year of hire.
· Completion of specialty courses in Advanced Cardiac Life Support, Pediatric Advanced Life Support, International Trauma Life Support, and Basic Life Support.
· Refer to Title 21, Sub Chapter 32H, Section .0501 of the North Carolina Administrative Code for specific certification requirements.
· Completion of ICS 700, 800, 100, 200, and 300.
Experience
· Minimum of two (2) years' experience at the Paramedic level.
· 1 to 2 years of experience in advanced emergency medical service work; or any equivalent combination of training and experience which provides the required knowledge, skills and abilities.
Knowledge, Skills and Abilities
· Knowledge that comes from required certifications and paramedic experience.
· Good interpersonal and customer service skills. Ability to relate to diverse populations within the community.
· Ability to read, write and communicate effectively in English.
· Proficient with MS Office and able to learn new software rapidly.
Valid NC Driver’s License: Yes
If driving a Wayne UNC Vehicle, must be 21 years old and MVR must be approved by Risk Management
