Job Summary:

In this role, you will function with the PATH Grant, focusing on both adult and youth outreach efforts. You will engage with individuals who frequently utilize emergency services due to unmet behavioral health needs and provide intensive support to youth and their caregivers in crisis situations.

Key Responsibilities:
· Engage and serve individuals experiencing homelessness, connecting them to ongoing supports and stable housing.
· Provide short-term case management to individuals utilizing emergency rooms and hospitals due to unmet behavioral health needs.
· Conduct intakes to assess services clients are eligible for and refer those who qualify for CPRC Community Support Services.
· Assist individuals with mental illness and those at risk of homelessness by providing consultations, resource information, and referrals to appropriate services.
· Network with community groups and organizations to meet individual client needs.
· Maintain a caseload of assigned PATH participants, addressing immediate needs and preparing for CPRC enrollment.
· Provide adequate documentation of activities as required by the funding agency and Brightli.
· Collaborate with hospital staff and community partners to reduce hospitalizations and ER visits for persons served.
· Conduct outreach services via community locations, homeless camps, and street outreach.
Education, Experience, and/or Credential Qualifications:
· Bachelor’s degree in Psychology, Sociology, Social Work, or other related human service specialized fields of study, including a BHS degree from an accredited institution.
· Advanced knowledge in mental health issues.
· Must have a valid Driver’s License and use a personal vehicle for transportation of consumers/clients. Must have auto liability insurance including uninsured motorist coverage.
Additional Qualifications:
· Successful completion of background check including criminal record, driving record, abuse/neglect, and fingerprint check.
· Completion of New Hire Orientation at the beginning of employment.
· All training requirements including Relias at the beginning of employment and annually thereafter.
· Current driver’s license, acceptable driving record, and current auto insurance.
Physical Requirements:
· ADA Consideration - Sedentary work: Exerting up to 10 pounds of force occasionally (exists up to 1/3 of the time) and/or a negligible amount of force frequently (exists 1/3 to 2/3 of the time) to lift, carry, push, or pull, or otherwise move objects, including the human body.
· Repetitive movements of hands, fingers, and arms for typing and/or writing during work shift.
· Sedentary work involves sitting most of the time but may involve walking or standing for brief periods of time.
