UPMC Living-at-Home Program is hiring a hybrid, full-time Outreach Social Worker, MSW!

Shift/Schedule: Full-Time Monday – Friday from 7:30am-4:00pm (Flexibility with scheduling is offered). Will be required to be on call for one week every other month.
***Daily travel to outreach sites required. UPMC Living-at-Home Program main office is 1501 Ardmore Blvd, Suite #404, Forest Hills, PA 15221; however, most of the time the staff member will be at outreach sites throughout Allegheny County. The staff member will also be spending 10 hours per week at Cedarwood Homes located at 1862 Broadhead Fording Road Pittsburgh, PA 15205.

Responsibilities:
Client and Volunteer Recruitment: Support the program director in identifying and recruiting clients and volunteers to participate in program activities
Outreach and Health Education: Design and implement outreach initiatives to promote geriatric care coordination in senior hi-rises and centers, including delivering health and wellness education
Community Presentations and Client Assessment: Organize and present (or coordinate presenters for) health-related lectures and workshops. Access client needs and refers them to appropriate services within UPMC and the broader community.
Program Evaluation and Reporting: Track and analyze key indicators to evaluate the effectiveness of outreach and educational efforts. Compile and maintain ongoing statistical reports.
Promotion of UPMC Services: Represent UPMC and its Community Provider Services at health fairs, screenings, and educational events. Develop a strong understanding of available clinical services to promote them effectively.
Service Coordination and Caseload Management: Provide service coordination at senior hi-rises as needed, including managing a small LAH social work caseload under the PHFA contract
Fundraising and Proposal Development: Assist the program director in developing proposals, meeting with legislators, and donors, and participating in fundraising activities to support program growth
Interdisciplinary Collaboration: Maintain regular communication with program nurses and social workers to ensure coordinated cared and service delivery
Professional Development and Quality Improvement: Participate in department meetings, staff development programs, UPMC-sponsored conferences, and professional in-service trainings. Contribute to quality improvement initiatives and maintain licensures and clinical expertise.
