Outpatient Therapist
**Job Description:** The Behavioral Health Therapist – Clinical is a licensed LPC, LMFT, or equivalent who utilizes clinical expertise to perform psychosocial assessment and develop/implement a plan of care/treatment plan that provides extraordinary care for the patient. This position works collaboratively with patients and the patients' family, healthcare providers, insurers, community resources, and other involved parties to identify patients' needs, provide services, coordinate care and transition plans, and advocate for patients. **Essential Functions** + Expertly applies trauma-informed care to build strong patient relationships, interpret behaviors, and respond effectively to patient needs. + Conducts psychosocial assessments and provides psychotherapeutic interventions in line with the treatment plan. + Administers assessment tools to screen for behavioral health symptoms and diagnoses. + Identifies patients at risk for delays, extended stays, readmission, or complex discharge needs and collaborates with care providers to find solutions. + Manages the discharge planning process, screens patient needs, develops discharge plans, and coordinates with internal and external services for post-treatment care. + Meets productivity standards and expectations. + Completes accurate and timely documentation in the medical record, adhering to departmental standards to ensure effective communication with the care team. + Satisfies all Intermountain Health requirements. Performs other duties as assigned. **Skills** + Child development + Behavioral Health + Customer Service + Interpersonal Communication + Trauma informed care + Family Education + Psychoeducation + Patient care + Cultural communication + Documentation **Minimum Qualifications** + Master's degree in counseling or related field from an accredited program (degree will be verified) + Current State Licensure as a LPC, LMFT, or equivalent + Excellent written and verbal communication skills + Proficiency in computer applications + Positive driving motor vehicle (DMV) record, proof of current automobile insurance coverage, and a copy of a valid and current Colorado driver's license + Basic Life Support and First Aid within 90 days of employment + Ability to adjust work schedule based on business needs. **Preferred Qualifications** + Experience working in a behavioral health setting + Bi-lingual in Spanish **Physical Requirements** + Ongoing need for employee to see and read information, labels, assess patient needs, operate monitors, identify equipment and supplies. + Frequent interactions with patient care providers, patients, and visitors that require employee to verbally communicate as well as hear and understand spoken information, alarms, needs, and issues quickly and accurately, particularly during emergency situations. + Manual dexterity of hands and fingers to manipulate complex and delicate equipment with precision and accuracy. This includes frequent computer use and typing for documenting patient care, accessing needed information, medication preparation, etc. + Ability to regularly lift and / or move up to 50 pounds. Employees must be physically able to use crisis prevention intervention techniques including: physical management of a child, running to prevent a runaway incident.
