The Department of Mental Health's Brockton Multi Service Center (BMSC) is seeking a licensed registered nurse to serve as an Outpatient Nurse (RN II) for the BMSC Outpatient Clinic. The identified candidate will be a part of a multi-disciplinary team that provides comprehensive services to adults individuals living in the community in a variety of settings. The identified candidate would operate under the supervision of the RN III Lead Outpatient Nurse; provides nursing services to assigned clients, responsible for providing injections to identified clients; liaison between community providers and outpatient psychiatry for assigned clients; responsible to be the contact person for assigned clients; provides nursing support to all outpatient clients; completes health assessments and other related screenings; provides routine and emergency medical care, ensures appropriate medical linkage in the community; provides supportive counseling; maintains a therapeutic atmosphere & professional performance; utilize databases, and performs related work as required. *_Please Note_*_:_This position is 40 hours per week on the day shift, 8:30 a.m. to 5:00 p.m., Days off: Saturday and Sunday. *Employees in this position will be required to perform mandatory overtime.* *_ Duties and Responsibilities (these duties and responsibilities are not all inclusive): _* * Participates fully in team meetings by coming prepared to present current information and promote the principles of recovery in collaboration with client’s and fellow team members. * Responsible for coordinating medical/psychiatric treatment for assigned clients to provide effective continuity of care. * Assess on an ongoing basis the needs of assigned clients by providing supportive counseling, by reviewing records and by maintaining regular contact with clients and families in order to ensure that appropriate recommendations for treatment are made. * Advocates for clients by acting as a liaison between them and the community to ensure that they are receiving appropriate services in the community. * Assist clients with scheduling medical and psychiatric appointments for assigned clients as needed. * Responsible for ensuring that assigned clients are encouraged to attend all needed and recommended physical, dental, & eye appointments and that written reports and orders are obtained in order to ensure that the client receives adequate medical follow up and care as needed. * Monitors vital signs, weights, labs and any other pertinent medical data to ensure abnormal findings are assessed and reported to the MD. Provide ongoing assessment of client’s medical/psychiatric status and intervenes appropriately on an urgent or routine basis as needed. Inputs REMS information for designated clients prescribed Clozaril within the outpatient clinic. * Assesses and responds to crisis and emergency situations by intervening immediately and contacting the Emergency Services Program, the Supervisor, and the other treating staff. * Completes required nursing documentation within designated timeframes and maintains the record of all assigned clients. * Provide medication administration to a caseload of outpatient clients who are ordered decanoate medications and assess their psychiatric/medical status * Follows all established agency medication transcription, storage, administration, and documentation practices. * Serves on committees to help share and improve service delivery at BMSC. * Assists with ensuring that all assigned clients’ medication reconciliation documentation is up to date and accurate. * Adheres to DMH and HIPAA regulations around privacy, confidentiality, and release of clinical information. * Promotes positive, courteous communication to clients, families, visitors, community providers, and agency personnel. * Responsible for Medication Refill Line coverage. * Involves all assigned persons served and significant others, when appropriate, in the process of treatment planning and ensure that plans are agreed upon by obtaining person served or LAR signature. * Responsible for developing & maintaining person served records by developing treatment plans, documenting progress or lack thereof and completing all other related forms to ensure compliance with DMH regulations, licensing & Joint Commission requirements. * Demonstrates the knowledge and skills necessary to provide appropriate care to the age related and cultural needs of the adult Persons served. * Provides primary nursing care by assessing health status, recording related health data, administering standard nursing treatment, evaluating persons served condition and outcome and adjusting care in conjunction with other service providers. * Meets persons served nursing care needs by communicating pertinent information to other involved staff to ensure integration, coordination and continuity of care. * Records and ensures implementation of psychiatry staff’s orders. * Provides nursing support surrounding persons served around major medical interventions. * Attends and participates in mandatory/required trainings. * Completes annual physical health assessments for designated clients. * Encourages clients to receive annual physical exams, routine medical and dental consultations and all health care services. * Performs other related duties as required. * * ** *_Preferred Qualifications:_*__ * Previous experience working with people with chronic mental illness. * Bi-lingual preferred. *_Brockton Multi Service Center Mission Statement:_* The Brockton Multi Service Center provides comprehensive services to mentally ill adults and children. Salary placement is determined by a combination of factors, including the candidate’s years of directly related experience and education, and alignment with our internal compensation structure as set forth by the Human Resources Division’s Hiring Guidelines.
