**Description:**
Trinity Health Of New England Medical Group is looking for a part-time Dietician to join a mission-driven healthcare team. Our vision is to provide state-of-the-art healthcare to those within our local communities, creating a positive impact for each individual that walks through our doors.
You would assist in coordinating the entire outpatient education and support group programs for the Comprehensive Weight Management Program including continuing education in nutrition for healthcare professionals. Provides optimal nutritional care to persons with weight issues by effectively identifying patient needs, assessing nutritional status, designing appropriate plans of care and monitoring patient progress to maximize clinical outcomes. Provides sound nutritional information/education and guidance to physicians, staff, patients, and families.
**What You Will Do:**
+ Oversees the development of weight management-related community education and support groups. Promotes positive relations with the community.
+ Develops and coordinates, in collaboration with appropriate health care specialists, curricula for various education programs as needed, including content, goals and objectives, target audience, instructors, evaluation tool, schedule, and charge. Keeps up-to-date on the latest educational materials and techniques to maintain state of the art education. Annually develops a procedure to evaluate the program curriculum for process, outcomes, and effectiveness.
+ Assesses nutritional status and educational needs of patients and documents assessments.
+ Provides nutritional counseling and close follow-up for patients and family members and documents educational plan.
+ Maintains communication with the referring provider about patient’s education plan and recommendations.
+ Participates in providing nutrition education activities and behavior modification skills in a group setting.
+ Represents Mercy Weight Loss Solutions, Mercy Medical Center, and Trinity Health Of New England as a professional dietitian to providers, patients, and the public.
+ Contributes to the goals for outreach and increased visibility and participates in building a referral base.
+ Participates in achieving and maintaining recognition of the educational program.
+ Participates in department and/or hospital quality improvement activities as requested.
+ Provides age-specific care related to patient’s unique physical and psychosocial needs.
**Requirements:**
+ **Education** : A minimum of 5 years experience in Nutrition Education required. Completion of a Bachelor’s Degree in Dietetics or closely related field. Master’s Degree in Nutrition Education, Public Health, Health Education, Counseling or related field preferred.
+ Registered Dietitian and licensed Dietitian/Nutritionist required.
+ Member of the Academy of Nutrition & Dietetics, American Diabetes Association, or American Association of Diabetes Educators.
+ Ability to effectively prioritize and execute tasks in a fast-paced environment. Excellent organization and communication skills required.
